چکیده

To the great disappointment of all concerned, too many operations performed for pulmonary carcinoma still prove to be exploratory thoracotomies or non-curative resections, in spite of careful preoperative investigations. A follow-up study of 168 patients operated upon for pulmonary carcinoma clearly shows that such non-curative procedures yield very disappointing results. To reduce this number of unnecessary and often harmful operations mediastinoscopy was introduced as an aid in detecting mediastinal spread of the tumour. It was thought that such knowledge would make preoperative assessment more accurate and so make easier the decision whether to operate.

The value of parasternal mediastinoscopy in staging bronchial carcinoma.

Computed tomography (CT) is the non-invasive staging procedure of choice for assessment of metastasis to mediastinal lymph nodes in patients with bronchial carcinoma. Cervical mediastinoscopy can provide histologic evidence of mediastinal spread to the peritracheal, tracheobronchial and subcarinal lymph nodes. Sub-aortic and para-aortic nodes cannot be sampled via this route. The present study ...

A retrospective study about 140 mediastinal lymphadenectasis patients receiving video - assisted mediastinoscopy

This study aimed to provide theoretical bases for choosing examination methods onpatients with mediastinal lymphadenectasis. The clinical characteristics of 140 patients with unexplained mediastinal lymphadenectasis who received video-assisted mediastinoscopy (VAM) were reviewed retrospectively. The relationship between various factors and pathological results of mediastinal lymphadenectasis wa...

Mediastinoscopy: a clinical evaluation of 400 consecutive cases.

Mediastinoscopy was carried out in 400 cases, including 296 of bronchogenic carcinoma. At the time of presentation the new growth had already spread to involve the mediastinal lymph nodes in slightly more than 50% of these. The incidence of involvement was 76% in oat-cell and 35% in squamous-cell carcinoma. Non-resectability at thoracotomy was encountered in seven out of 120 patients. We advoca...

Endobronchial ultrasound-guided transbronchial needle biopsy for M1 staging of extrathoracic malignancies.

BACKGROUND extrathoracic malignancies metastasize to the mediastinum and/or pulmonary hilum. Mediastinoscopy and thoracoscopy are standard to obtain tissue proof of metastatic spread but are invasive. Endobronchial ultrasound with real-time-guided transbronchial fine-needle aspiration (EBUS-TBNA) is a minimally invasive alternative for surgical staging of lung cancer. METHODS we analysed the ...

Minimally invasive esophagectomy via Sweet approach in combination with cervical mediastinoscopy for esophageal squamous cell carcinoma: a case series

Objective Minimally invasive esophagectomy (MIE) is increasingly used for the treatment of esophageal cancer. However, MIE via the Sweet approach has seldom been reported owing to the challenging procedure for a mediastinal lymph node. Thus, the approach of MIE via left-sided thoracoscopy coupled with video-assisted cervical mediastinoscopy (MIE-SM) was explored for eradicating the mediastinal ...
